Adult Care Facility Resource Information

In 2013, NAMI Ohio was asked to assist the state of Ohio in developing a comprehensive listing of adult care facilities (ACFs)  and the services they offer.  Specifically, we sent a survey to each facility licensed by the state and asked for information such as the types of individuals they serve (male, female, age, etc.), special needs accommodations, medication assistance, smoking policies, staffing, transportation assistance, and more.  The information was to be added to an existing interactive website in which users are able to search a database for possible nursing homes for their loved ones.  Unfortunately, after NAMI Ohio collected the information, we learned that the state had decided not to expand the website to include ACFs.  Therefore, we are making this information available through our own website.  However, users are forewarned that the information contained in this spreadsheet was collected over two years ago and may no longer be accurate.  We are making it available because we believe that some information is better than no information.  Please contact the home directly if you have any questions regarding the services they offer or space availability.
